Where, Why, How, What year, and Who banned THE BOOK?

I need to know how this banned was banned and who banned it and what year it was banned and where it was banned.. the book not the movie.

There are quite a few cases in several states that have banned the book from high school libraries and classrooms:

Aurora, Colorado (1976)

Westport, Rhode Island (1977)

Anniston, Alabama (1984)

As for why the book was banned, it depicts rape, other violence, and unsettling language.
