How can the reader tell the old man is recovering from an illness.

I don't think that it is stated that the old man has an illness. He certainly looks in bad shape though,

...old man, a very old man, lying face down in the mud, who, in spite of his tremendous efforts, couldn't get up, impeded by his enormous wings.
