How does Dr. Warren fix Johnny's hand?

Dr. Warren believes that he can do surgery on Johnny's hand and cut the thumb loose from the constricting scar tissue.  He believes that Johnny will likely never be a silversmith again; he will, however, be able to fire a rife and fight for his country.
