its chapter 12 starts with "a visiting doctor covered in gray cobwebs...." the question is referring to the lastparagraph of this short chapters tarting with "out in the staff room the visting authority..."
THe Chief is recalling his own experiences in the mountains fishing...as channeled through the picture on the wall. The young skinny doctor personifies Chief's experience. He is shivering and his clothes seem to "flap around his bones." Although Chief and the doctor are not having the same experience the irony is still there.
