Dr. Spivey is the antithesis of nurse Ratchet. McMurphy is able to manipulate the doctor into whatever he wants. This not only tells the reader about McMurphy's innate intellect, it shows his keen powers of manipulation. McMurphy is able to play off Spivey's insecurities goading him to stand up to the nurse. I'm not sure which specific scene you are referring to with the glasses.
