One flew over the cuckoos nest
The idea is that the Chief lives in a kind of "fog" - in other words he feels safe when he can only see what affects himself and nothing outside of him. In the same way, the fog makes him feel safe - he cannot see beyond himself and thus avoids the outside world.
The fog insulates the chief and other patients from the world. Often Chief goes to a different place in his mind.
