What  does Pavel mean when he says, 'just because a man glances up at the sky at night does not make him an astronomer, you know'?

Pavel does not want anyone to find out he is actually a skilled doctor.

He means that appearances can be deciving. That even though pavel looks like a normal house worker, he's got a pasion doing something that could save a life. Just like you may think that someone looking at the stars could be an astronamer but he could be someone completly differnt.
