Why is the word "it" in italics? What does that have to do with Psychology of death?

"It" is in italics to emphasize Ivan's preoccupation with death. "It" is death. The italics are meant to convey Ivan's feelings that "death" is a living thing, as it encroaches upon his thoughts and becomes his obsession.
