After Kino shows the servant the eight ugly pearls he has as payment to the doctor, the servant leaves and returns right away. Why do you think the Servant returned so soon.

The servant returned to tell Kino the doctor was out. In my opinion, he returned quickly because he (the servant) didn't even need to tell the doctor..... he already knew that the payment was unacceptable.
