How did the doctor treat kino and Juana when they first came to see him?

The beggars no the doctor is useless to them. They know he only cares about money and will not treat people who don't have it. The Doctor would rather be relaxing in Paris. The doctor thinks Kino and his people are poor savages who cannot possibly afford his care.
