They have not had a doctor and they wish for one. They also want him to help people like Hester (who might confess her sin to him). The townspeople do not realize that he will torture Dimmesdale under the guise of helping him with health issues. In fact, he creates some of Dimmesdale's health issues by the way he tries to control him.
