Who is to blame for Dianna’s death?

Dianna is seeing another man despite being a married woman with twins. Her current husband is not aware of her infidelity. Dianna meets her death while she is seeing the other man. She is involved in a car crash and she dies instantly. The twins are blaming themselves for their mother's death but the reality is that they have nothing to do with her death. Dianna's side lover is also not to blame because it was not his wish that Dianna dies in a car crash. Therefore, Dianna's death is caused by an accident and the blame should not be directed to anybody.

The entire book is full of duplicities implying that there is a lack of trust among the characters. For instance, Dianna betrays her husband by having a secretive affair with another man. Dianna claims that she is not in love with the father of her twins but she is deeply in love with the secretive lover. Her husband is not aware that his wife is seeing another man. Disloyalty is also evident between the twins because they are taking advantage of each other for egotistical gains.
