What do you think “carers” do? Why do you think the author doesn’t tell you?

In the first section, we learn that carers are responsible for "donors," some of whom may be childhood friends and companions. Being a carer (or a donor) seems to be compulsory, and that those who must carry out these duties went to certain schools. Most importantly, the inevitable fate of a carer is to become a donor. Allow students to speculate on what donors might be donating, based on the details of the "recovery centres."
