What sort of shift does the wife’s doctor undergo during today’s reading?

Up until this point the doctor's wife feels that she has the responsibility to help those that cannot help themselves. She realizes, though, that this is simply not possible. Even if she did tell the people that she could see, she would more likely than not merely end up their slave as opposed to being able to help them.
